Forest City Behavior offers an array of psychological assessment services including, but not limited to:

  • Developmental disability determination evaluations for Social Security and the Illinois Developmental Disability Medicaid Waiver.

  • Diagnostic assessments for Autism Spectrum Disorders and other developmental disabilities.

  • Independent intellectual and developmental disability evaluations for assistance with educational planning.

  • Level of Functioning assessments to meet requirements for periodic updates for people receiving Medicaid Waiver-Funded services.

  • Comprehensive evaluations of intellectual and adaptive functioning to aid in long term treatment planning.
